People In Need
AVMF is committed to a compassionate and caring response to helping the most vulnerable and needy members of our community. Direct Assistance funding is provided by AVMF for people with needs for dental procedures, prescriptions, emergency medical services, and mental health care.
REQUESTS TO AVMF MUST NOT COME DIRECTLY FROM THE PATIENT NEEDING HELP. ONLY A NON-PROFIT ORGANIZATION, GOVERNMENTAL AGENCY OR HEALTHCARE PROVIDER MAY SUBMIT REQUESTS ON BEHALF OF THE INDIVIDUAL IN NEED. Once approved, payments by AVMF are made to the healthcare provider or approved pharmacy.
Individuals never receive financial assistance directly from AVMF.
Policy and Procedures
• AVMF must approve an application before any funds are committed for patients. Pre-approval is required
• Please FAX the printed form or email to AVMF for consideration.
• Do not have patients contact AVMF under any circumstances.
• A maximum of $150 per person per year is the generally the maximum assistance for dental, vision and prescription assistance.
• Mental health assistance is provided within certain guidelines.
• Providers receive payment directly from AVMF.
